Important Information
Waiting Periods
- 3 months - Main member and family
- 6 months - Parents and Extended family (natural deaths)
- Immediate - Accidental death (with 1 month premium paid)
- 24 months - Suicide deaths
Claim Requirements
- Completed claim form
- Certified death certificate
- Certified copy of claimant's Omang
- Marriage certificate (spouse claims)
- Police report (accidental death)
- Proof of relationship where surname differs
- Completed KYC form
- Proof of bank account
Membership Note: Membership subscription fee is 1% of member's basic salary (capped from P50.00 to P120.00). The scheme is underwritten by Babereki Life Insurance Company (BLIC).
